Using the button above to begin the registration process for the Patient Portal. You will enter your email address and create a password that will allow you to login into the portal in the future. After creating your login, you will enter your personal information. Once you’ve complete the registration steps, you will be taken to the portal dashboard. From there you can begin using the portal to complete your Wellness Snapshot or schedule a primary care appointment.
