How to make an appointment for a syringe?

To help provide you the best possible care, please bring the following with you to your appointment: 1 A list of current medications (bring the bottles themselves if you are unsure) including vitamins or other over-the-counter medicines 2 For children’s visits, please bring your child’s immunization records 3 A list of questions you want to discuss with your provider. If you have any questions, you and your provider may select those that are most important and make a follow-up appointment to address the rest 4 Your insurance card or coverage information 5 Your co-payment (if required by your insurance.) Yakima Valley Farm Workers Clinic serves more than 181,000 people across 25 medical clinics, 14 dental clinics, 9 pharmacies, and 53 programs in two states, Washington and Oregon. We have expanded medical care to include dental, orthodontia, optometry, nutrition counseling, autism screening, and behavioral health services. We also offer community programs that offer assistance with employment and training, afterschool education, rental and shelter assistance, energy assistance, weatherization, HIV and AIDS counseling and testing, home visits, tobacco cessation, and three mobile medical/dental clinics.
