MyChart offers patients personalized and secure on-line access to portions of their medical records. It enables you to securely use the Internet to help manage and receive information about your health. With MyChart, you can use the Internet to: Request medical appointments. View your health summary from the MyChart electronic health record.
MyChart Patients Chart Self Scheduling COVID Vaccination This document will show a patient the steps that will be taken in order to schedule their COVID-19 Vaccine
MRN is short for Medical Record Number. This number is an nine-digit number including any leading zeros. It is a unique number assigned to you when you first see a doctor at WellStar. You can find your MRN on your bill statement, after visit summary, appointment schedule reminder, and appointment reminder letter.
You can also download the MyChart app for your smart phone. For iPhone and iPad: Visit the iTunes app store to download the MyChart app. Then select WellStar Health System as your provider. The app is also available for Android.
Wellstar will comply with this mandate starting March 17, 2021.
As a federal statute, all health systems are required to comply with the Cures Act by April 5, 2021. Building on our existing commitment to transparency with our patients, our goal is to strengthen our patient/clinician partnerships, enhance your experience and ultimately provide better care.
To request medical records, you must be the patient, or the parent or guardian of the patient for whom you seek records. Caregivers may be able to access records if the patient has provided written permission to the provider.
If you wish to obtain medical records for you personal use we are happy to assist you in this process. The per page charge is as follows: 1 Electronic records delivered in electronic format—$6.50 2 Medical record with paper records delivered in electronic format are billed at $6.50 + 7¢ per page labor cost to create and deliver the portion of the record maintained in paper. 3 Electronic records delivered in paper 90¢ labor cost to create and deliver the portion of the record maintained electronically plus 5¢ per page for paper and toner. 4 Paper records delivered in electronic format 7¢ per page labor fee. 5 Paper records delivered in paper 12¢ per page.
Once you get the PDF, you can save it to your personal computer. If you do not have an active MyChart account with Wellstar: On the MyChart main web page, click on Sign Up (I don't have a code). Complete the requested information and click Submit.
If a provider believes that letting you look at your medical records can endanger your physical health, your request may be refused. They cannot deny you access just because they think you will be upset, unless they believe revealing the information might lead you to physically harming yourself.
Ask your care team about MyChart Bedside. This touchscreen tablet is specifically designed to keep you connected to nearly every aspect of your inpatient care plan. As a Wellstar hospital patient, you’ll be able to keep the secure, easy-to-use tablet during your entire stay.

Use soap and water or alcohol-based hand sanitizer under your nails, in between your fingers, and on the palms and backs of your hands. Rub for 20 seconds (the time it takes to sing “Happy Birthday” twice).
Family and visitor lounges are on each patient floor and on the first floor of the hospital. Specific areas have been designated for families of patients in the Intensive Care Unit, the Emergency Department and Surgery. Wi-Fi is available in these areas.
If you are unable to present any evidence of healthcare insurance coverage, you then will assume full financial responsibility for payment of all the charges incurred during your stay. A representative from the Patient Access Services Department will help you establish appropriate financial arrangements. In some cases, based on the information provided, patients may apply for other types of financial assistance programs.
We will need a copy of your Medicaid card. Medicaid also has payment limitations on a number of services and items. Medicaid does not pay for the cost of a private room, unless medically necessary.
If a transfer is recommended, you will be informed of the benefits and the alternatives. You will not be transferred without your consent and until the other institution and/or physician agrees to accept you.
