It is branded to the health organization and is available as a web application, iOS app, and Android app. Bridge Patient Portal is much more than a traditional patient portal, offering a suite of patient engagement features from appointment scheduling to bill pay to care plans.

The Bridge allows members of the patient’s care team to exchange medically relevant messages between regular visits. Patients or registered family members receive a health update through the Bridge after every visit. Through the Bluestone Bridge, you can communicate directly with your provider team. Access to the Bridge is through the purple button at the top of the site.

How to Get Started

Fill out the My LifeBridge Health online request form, which can be downloaded here. You will receive an email with a link and instructions to complete your registration.

API Request

We offer the ability for you to securely connect some of the health management apps you may use (i.e. fitness trackers, dietary trackers, etc.) to your health record through your patient portal account. This allows for easy access to health and wellness data, enabling you and your care team to make informed choices.
