Before Your Visit

If it’s your first visit, the provider will want to review your medical records, medication lists, test results and films (X-rays) to understand your prior medical history and avoid unnecessary testing. When you make your appointment, ask the appointment secretary for a request of medical records form to have your records sent to our clinic.

During Your Visit

You will meet with a nurse who will ask for the information needed before you are seen by a provider, a doctor, a physician assistant (PA) or a nurse practitioner (NP). You may need additional testing, such as blood work or imaging (X-rays, scans, etc.), to help your doctor better understand your condition and decide on the best treatment for you.

After Your Visit

Your follow-up care will be determined based on your individual needs. You will receive information about how to contact us if you have any questions or concerns after you leave our clinic. We look forward to seeing you. If you have any questions prior to your visit, feel free to contact us.
