San Luis Valley Health transitioned to a new patient portal experience with the new Electronic Medical Record (EMR), MEDITECH . Step One: Please contact Medical Records or any SLVH admissions representative (any clinic or hospital), and provide them with your email address, date of birth, and phone number.

Your portal history will not transfer over to the new portal. The history and content from the old portal will be available until December 31, 2020.
