How do I get my medical record from Union Hospital?

If your appointment was through Union Hospital’s outpatient physician network (Union Physician Services), you will have access to your electronic medical record through Cleveland Clinic My Chart. If you were admitted to Union Hospital, please use Union Hospital’s Patient Portal to gain access to your electronic medical record.

What can I do on the patient portal?

This portal allows patients to view their medical records, request refills from their providers, make appointments and communicate via secure messaging with their provider and clinic staff. Check with a staff member at your provider’s office to assist you in obtaining your username and password.

Minor: Parents may have access to minor patient records (0-17) by contacting the Medical Records department for Patient Portal Proxy form and present a copy of ID.

Go to the Medical Records department in person and ask to enroll in Patient Portal.

How long is the enrollment email active?

Please use the temporary user ID, password, and link provided to log in to Patient Portal. Please note that the enrollment email invitation is only active for 14 days from receipt.

What is the phone number for Health Information Management?

Call Health Information Management at 330.343.3311, ext. 2427. Please call during regular business hours. You will be asked for your email address and other identifying information. Once your ID is confirmed, you will receive an email message with instructions to set up your Portal account.

DELIVERIES

Mail, flowers, and gifts are delivered daily to patient rooms by our staff of volunteers. Regulations prohibit live flowers in any Critical Care room. Mail received at the hospital after your discharge will be forwarded to the address provided on your admission form.

PATIENT EXPERINECE

Providing superior care and service to each one of our patients and their guests is important to us at Memorial Hospital. At any point during your stay if you feel we are not living up to this expectation, the Patient Experience Department would welcome that feedback so we can make the necessary improvements.

MEALS & FOOD

Complete meals or snacks are available for your family and visitors in the Bistro ‘52 from 6:30 am to 6:30 pm, seven days a week. Vending machines throughout the hospital offer sandwiches, snack foods, candy, and beverages 24-hours a day.

MONEY & VALUABLES

Please send money, jewelry, and other valuables home with your family. If this is not possible, your nurse will arrange to have valuables stored in the hospital safe. The hospital is not responsible for money, jewelry, clothing, dentures, glasses, or other valuables kept in your room.

NEWSPAPERS

Memorial Hospital inpatients will each receive a copy of the local newspaper with their evening meal tray on Monday through Saturday. Additional copies are distributed in many of the department waiting areas.
