How long does it take to respond to an emergency call?

The anticipated turnaround time for response to electronic messages is 48-72 hours, Monday - Friday, 8 a.m. to 4:30 p.m. For all urgent medical matters, contact your physician's office by phone, go to an emergency room, or dial 911. If you have a medical emergency, immediately dial 911.

Should I change my password if I have a compromised account?

You should prevent disclosure of your activation code, ID or password, and you should change your password if you believe that your security may have been compromised. In addition, you should change your password regularly to help maintain the security of your account.

How long does it take to sign up for UnityPoint?

Anyone 18 years of age or older and is a patient of UnityPoint Health can register to use MyUnityPoint. It just takes five minutes to sign up, and your account will start within 1 to 3 days.

How to send a secure message to a patient?

Secure patient messaging allows you to: 1 Request a medicine refill 2 Ask a question about your lab results 3 Ask a question about your symptoms, treatment plan or medicine 4 Request an appointment 5 Provide information your care team has asked for (such as blood pressure, weight or blood sugar level) 6 Ask any non-urgent questions or concerns 7 Provide information or ask questions about your child’s health (with proxy access)

Can I access my children's health records?

MyUnityPoint can provide access to your children’s health records too. Proxy access allows users to see health information for their children age 11 or younger, and for anyone age 12 and older they have the legal right to make health care decisions for (legal documents are required). How to Register for MyUnityPoint

If you don't already have access to our patient portal, register for MyUnityPoint by visiting chart.myunitypoint.org. Request an activation code and it will be sent to your email. It may take up to a week for you to receive the code. If you need assistance, call MyUnityPoint Support at (877) 224-4430, or email MyUnityPointSupport@unitypoint.org.

Authorization Form

An authorization must be completed in its entirety to receive a copy of your medical records. Please mail the authorization back to the HIM Department at the appropriate address listed above. There may be a fee based on the type of request. Media formats available include paper, CD and DVD. Postage fees will apply if the request needs to be mailed.

Request for Birth Certificates

If you need your birth certificate, please contact the Iowa Department of Public Health, as we do not have copies of birth certificates.
