How do I contact University Health Services (UHS)?

What is proxy access in UHS?

One of the features of MyChart is Proxy Access. Obtaining access to another persons’ UHS MyChart account that is NOT their own is called Proxy Access.

How to get my MRN number?

Get your medical record number (MRN) and activation code by calling the Patient Portal Help Desk at 734-615-0872 (7:30 am - 4 pm). Then create your account at MyUofMHealth.org by clicking the blue "Sign Up Now" button.

Do you have to be registered in the UHS electronic health record?

For all others: You must be registered as a patient in the UHS electronic health record (MiChart) in order to create a portal account.

Students: Document Your Vaccination

If you are a student and have received a first dose of the Moderna or Pfizer vaccine, upload a picture of your vaccine card to your UHS patient portal. (Use the "My Forms" feature and click "COVID Vaccination - Students.")

Faculty & Staff: Document Your Vaccination

If you are a faculty, or staff member and you have received the second dose of the Moderna or Pfizer COVID-19 vaccine or a single dose of the Johnson & Johnson COVID-19 vaccine, the University asks that you upload your vaccination documentation into PyraMed, the electronic medical record in University Health Services.
