Benton Family Clinic welcomes you to our practice. Once you have made your initial appointment, please read through the information below to help streamline your first visit with us.
From Interstate 30 W: Take exit 117 for AR-5 toward AR-35/Benton. Merge onto AR-5 S/I-30 Frontage Rd. Turn right onto AR-5. Use the left two lanes to turn left onto AR-35 N/AR-5 N. Continue straight on W Carpenter St for 0.1 miles. After crossing over the bridge, your destination will be on your left.
You are able to upload a document to the patient portal. In the "Messages" section, select Compose New. You have the option to select the message type. Select Patient Attachment and the upload icon will appear. By selecting upload, you can attach your file. There is a file size limit of 800kb and only the following file types can be attached: .tif, .tiff, .jpeg, .jpg, .jif, .jfif, .jp2, .jpx, .j2k, .j2c, .fpx, .png, .pdf, .doc, .docx, .dot, .dotx, .text, .txt, .xls, .xlsx, .xla, .xltx
Statements are located under the "Billing" section of the patient portal. Look directly under "Welcome" on the far right. You can view statements there.
Select Medications from the "CHART". On the line of the medication you wish to refill, select "Request Refill". Please make sure to select the appropriate dose and pharmacy for this medication. Please DO NOT send refill request if your annual physical is not scheduled or up-to-date.
Education forms are located under the "CHART" . Once in the Chart, along the left hand side you will see a list of options. Select EDUCATION MATERIAL to view any material your provider has attached to your chart.
Select "Visit Summaries" from the "CHART". To the right of the visit date, you will have the ability to "View Summary" or download to your local machine.
For after hours medical questions or concerns, please contact the on-call provider by calling our answering service on our main line, 303-948-2676. For routine medication refills, we prefer to receive those electronically from your pharmacy or through the portal. Or, please contact your pharmacy directly.
Due to HIPAA regulations, we cannot and will not respond to patient portal messages unless the question is sent from that patients chart. For example: Wives, please do not message us from your patient chart about your husband's healthcare. This is a violation of privacy and the message will be immediately stricken from your chart.
