Do you need to schedule a routine appointment or follow-up visit? Log into the Patient Portal, send us a message, and we’ll take care of it. If you have lost your login or don’t have one yet, please call our office at 850-878-8714 and we’ll be glad to assist you. A referral is not required to schedule an appointment.

How to receive follow my health portal invitation?

To receive an invitation for the Follow My Health patient portal please call your physician's office to have one sent to you.

How long does it take for a lab to show up on a portal?

It can take up to 72 hours for lab results to show in your portal account. Other information should show the same day. You will not be able to see radiology results, such as x-rays and MRIs. For a complete copy of your medical record please contact your physician’s office and ask for medical records.

Does TMH include hospital records?

Your information is obtained from your TMH Physician Partners outpatient electronic medical record. It does not include hospital records.
