What is a patient portal?

To sign up for the Patient Portal, please download and complete the Patient or Representative Authorization Form, and submit to the Business Office via fax to (631) 477-8262 or via email to Kathryn.Reich3@stonybrookmedicine.edu . What is a physician portal?

The patient portal is a secure online tool that will help patients communicate with our office (The privacy and security of your medical information, is important to us. You can view Stony Brook Ophthalmology's HIPAA Privacy Notice, which follows the Health Insurance Portablility and Accountability Act of 1996 (HIPAA).

Patient Portal LOG IN

Stony Brook Southampton Hospital's Patient Portal is a convenient, secure, online tool available 24/7 that will help you view, download, or transmit many of your hospital health records. You can enroll at the time of hospital admittance. Your medical information will be kept secure, per HIPAA regulations.

Once you have a Stony Brook Southampton Hospital user name and password, please log in here. This portal serves only Stony Brook Southamton Hospital patients. If you are having trouble, please contact us .

What health information is available in the patient portal?

The Patient Portal currently holds laboratory, radiology results for all visits and transition of care documents for Observation, Inpatient and Emergency Department visits, starting with the first visit after a patient has registered for the Portal. Only health information transmitted after enrollment will appear in the Patient Portal.

If you have an upcoming appointment, you must contact the office to review our protocol during the COVID pandemic.

COVID-19 Office Visit Policy

You should come ALONE to your appointment. If you feel you need assistance at your appointment that person should be symptom free of any illness. Drivers should remain in the car. No one except the patient will be allowed in the waiting room.
