Pediatric Medical History

All new patients under 17 years of age will need to bring this form in filled out to their first appointment.

Adult Medical History

All new patients 18 years of age and older will need to bring this form in filled out to their first appointment.

Prescription Refill Policy

Anytime you are out of refills, please call your pharmacy to speak to the pharmacist (not the automated pharmacy phone line) and tell him/her that you are out of refills. The pharmacist will then contact your physician via fax or email with your refill request.

Jennifer Ligon-Gaston, FNP-C

Jennifer Ligon-Gaston, FNP-C is a board-certified Family Nurse Practitioner. She has over 17 years combined experience including general surgery, general orthopedics and spine surgery, plastics and reconstructive surgery, obstetrics, gynecology and family medicine. She joined Stone Creek Family Medicine in 2016.
