Make an Appointment

To request an appointment, call us at (865) 632-5885 Monday-Friday, 8:30 a.m. until 4 p.m. Please call at least 24 hours in advance for appointment cancellations or rescheduling.

Parking

We are located at 6600 Nightingale Lane, Knoxville, TN 37909. Ample parking is available at our private office building. Patients seeing one of our providers should park in the top lot adjacent to the main entrance door.

New Patient Registration

On your first visit, you will be asked to fill out registration forms. You may choose to pre-register by downloading, printing and completing the forms below. Just bring them to your first visit. Completing the forms prior to your visit will help speed up the new patient registration process.

Insurance

Southern Medical Group participates with Medicare and most major HMO and PPO insurance plans. As a courtesy, we will file all claims with your insurance company. If you have questions regarding your insurance coverage, please contact us at (865) 632-5885.

Prescription Refills

It’s very important that you contact the office during regular office hours for all prescription refills. We are unable to refill any prescriptions after 5 p.m. Monday through Friday, and we are unable to refill any prescriptions on weekends or holidays.

How long does it take to log out of MyChart?

We aim to protect your privacy and security of your information. While logged into MyChart, if your keyboard remains idle for 15 minutes or more , you will be automatically logged out of MyChart. We recommend that you log out of your MyChart session if you need to leave your computer for even a short period of time.

How long does it take for a test to be released on Summit Health?

Test results are released to a patient’s Summit Health MyChart account after their provider has reviewed them or after 3 days regardless if the provider has reviewed the results. Tests that are sensitive nature may not be released to Summit Health MyChart.

Where does MyChart come from?

Your MyChart information comes directly from your electronic medical record at your doctor’s office. Ask your doctor to correct any inaccurate information at your next clinic visit. Your health information is reviewed and updated in your electronic medical record each visit.

Is MyChart secure?

Each person controls their password, and the account cannot be accessed without that password. Further, MyChart uses the latest 128-bit SSL encryption technology with no caching to automatically encrypt your session with MyChart. Unlike conventional e-mail, all MyChart messaging is done while you are securely logged on to our website.
