New patients should arrive 15 minutes prior to the scheduled appointment time. If we have snow, ice, or other extreme weather, please call our office first to confirm your appointment.15
Please call us in advance to change or cancel your scheduled appointment. Cancellations made less than two business days prior to your appointment may result in a cancellation fee which is not covered by health insurance ($50 for a new office visit and $25 for a follow-up visit).
