Starting, Monday, March 15, 2021, The Polk County Health Department, Polk County hospitals and clinics and select Medicap pharmacies will open COVID-19 vaccine appointments to 64 years and under with underlying health conditions pending vaccine allocation. Get vaccinated and register today for your chance to win $$$!

Free services to women ages 35-64 who are uninsured. Services include: Pap Smears and Mammograms in addition to the above.
Program provides annual health screenings for women ages 40-64 including blood pressure, cholesterol, HDL, and blood glucose. Free gym memberships and Weight Management programs are offered as available.
