How do I request medical records in Arizona?

Medical Record Request This Medical Records Request form is strictly for patients or treating physicians. If you are not the patient or referring provider please fax your request on letterhead paper to (520) 326-7989. *If you need images within 48 hours, please call the Medical Records Department at (520) 545-1822 after submitting this request.
