Medical Records

For new patients, PDEC will attempt to get pertinent medical records from the referring provider. Sometimes, there is a delay in this process. It is helpful if the patient assists us with this process by arranging to have relevant records sent to our office in advance to their appointment date.

Insurance

PDEC accepts most insurance plans. If you have specific questions regarding your insurance, you can contact our billing department at (503)274-4808. Please note, it is the patient responsibility to inform our office of any changes in insurance coverage.

Patient Payments

PDEC accepts cash, personal checks, MasterCard, Discover, Visa, and American Express. Checks can be made out to Portland Diabetes and Endocrinology Center or PDEC. It is the policy of PDEC to make all reasonable attempts to collect outstanding balances’ should they accrue, including, convenient payment plans.

Appointment Cancellations

If a patient must cancel, PDEC requires a 24-hour notice. PDEC may charge a $25.00 fee to patients that cancel or miss their visit without a 24 hours’ notice to us.

No Show Policy

A “no show” is someone who misses an appointment without canceling it within one (1) business day in advance. No-shows inconvenience those individuals who need access to medical care promptly. A failure to present at the time of a scheduled appointment will be recorded in a patient’s medical chart as a “no show”.

Current List of Medications

Patients are to bring a complete list of their current prescribed and over-the-counter medications, vitamins, and supplements to their first appointment, including dosages. Patients will be asked to notify their provider of any changes made at future appointments.

Instructions for Patients with Diabetes

Patients should always bring their blood glucose meters and/or hand-written blood sugar records to all appointments. If using a CMG or insulin pump, PDEC may ask you to have our current log-in information to be linked to PDEC. All of these devices will be taken at check-in, so that they may be downloaded for review.

Born and raised in San Diego, California, Dr. Grady grew up in a pair of shorts. He attended UCLA where he majored in micro and molecular biology while learning some French along the way.

James L. Neifing, MD

Dr. Neifing attended the University of Illinois in Urbana-Champaign where he received both a Bachelor’s degree in Electrical Engineering and a Master’s in Business Administration. He received his Medical Degree in 1986 from the University of Illinois College of Medicine.

Amy J. Carter, FNP

Amy Carter is a Family Nurse Practitioner specializing in the treatment of patients with endocrine disorders. She received her Bachelor of Science in Nursing from the University of Hawaii at Hilo and her Master’s Degree in Nursing from Washington State University with a specialty focus on diabetes and endocrinology.

Karen A. Jacobson, PA-C

Karen Jacobson is a Certified Physician Assistant specializing in the treatment of patients with endocrine disorders. She attended Bowdoin College and majored in Biology. She received her Masters of PA Studies from Oregon Health and Science University.

Jennifer L. Murdoch, ANP-C
