Starting June 22, 2021, CentraState’s physician practices will be moving to a new electronic health record system and new patient portal, MyChart. At that time, access to your practice’s portal account will be limited to medical records for visits that occurred before June 22.
You can register for a MyChart patient portal account at the time of your next appointment. Our staff will send you a registration link via a secure email. What can I do to take ownership of my health? 1 Make a list of all the specialists you have seen since your last visit. Bring any reports you were given with you to your visit. 2 Make a list of all the medications that you are taking, including over-the-counter medications, vitamins, supplements and herbal remedies. Bring the bottles with you to your visit, if possible. 3 Make a list of any changes in your Medical History or the Medical History of family members. 4 Make a list of 3 health goals for yourself and talk about them with your doctor to form a Care Plan
For urgent matters after-hours, there is always an on-call physician available at (401) 272-3410. Please leave a voicemail message for the on-call physician and he will respond within 1 hour. For emergencies, always dial 911.
