The health dashboard is a place where you can access your health records including COVID-19 test results and vaccination cards. You can also type in your symptoms and let us guide you to the right care options.
If we have your email address on file, then your MyChart username will be sent to your email account. If your email address has changed, please call MyChart Services at 1-855-551-6555.
If you do not remember any of this information, or you do not have a valid e-mail address on file, you will have to contact your MyChart help desk at 1(866)385-7060 to help you regain access to your MyChart account.
DEACTIVATION: Participation in MyChart is voluntary. Users may request deactivation of an account by contacting the MyChart patient helpdesk at (877) 768-0732. We reserve the right to deactivate or prevent a user's access to any or all MyChart features at any time for any reason.
