Proxy Access to Your Adult Family Member's Health Information

If you are seeking proxy access to an adult family member's My Patient Connect account, and you are not the legal guardian (or active DPOA) of that person, both parties are required to complete and sign an authorization for proxy access form and submit it to our Health Information Management Services Department via patientportal@crhc.org.

As a parent or legal guardian of a minor, you're permitted access to your child's health information until your child reaches the age of 12. At that time, your access is automatically removed, pursuant to NH RSA 318-B:12-a and NH RSA 141-C:18.

Parents can have electronic access to their child’s health records until the child is 12 years old. On the child’s twelfth birthday, electronic access will automatically be removed. Parents cannot obtain electronic access to their child’s health record if the child is 12-18 years of age.

Yes. My Patient Connect uses highly secure encryption technologies with no caching. This means My Patient Connect will not record any of your patient information on the computer or mobile device you are using to access your My Patient Connect account.
