NBMC has been in the business of caring for Coos County residents since we opened our doors in 1972 at our original North Bend Clinic. In 1978, we continued the tradition of providing excellent care to our patients by opening a brand new clinic in Coos Bay. Today, our 120,000 square foot, modern facility houses many services, including: primary care, specialty care, a full-service lab, day surgery, imaging department and the Immediate Care Clinic. We are also conveniently located on the same campus as Bay Area Hospital and the Bay Area Cancer Center.

The new North Country Healthcare patient portal is an online tool that goes a step beyond simply displaying information by helping you manage your health care easily and securely. All of the information in the portal comes from your North Country Healthcare electronic health record — from all North Country Healthcare facilities.

The patient portal is a website that allows you a convenient and secure way to view portions of your medical records and communicate with your provider’s health care team. Within your portal, you will be able to do the following:
Northstate Medical Center in Roxboro, North Carolina is a pediatric and internal medicine practice. Our physicians also provide family medicine. Doctor Patrick Godwin, MD, Dr. Roberts Smith, MD, Dr. James Winslow, MD and Family Nurse Practitioner Tabatha Horner, FNP have extensive experience. Medical services include physicals, wellness exams, flu shots, vaccines, sports physicals, spirometry, primary care, allergy testing, drug screening, pre-employment physicals, hearing tests and diagnostic tests including EKG and ICG (take home heart rate monitoring), laboratory testing, stitches and wart treatments. Children under 12 years of age and dependent adults who have a Dignity Health doctor may have a portal with an adult as the proxy. Accounts are not available for adolescents 12 – 17 years of age. For a dependent adult, they will need to sign a release, or you will need to provide a copy of the Medical Durable Power of Attorney before an account can be created for the dependent adult.
Some of the information in your portal is provided through your medical health record and is viewable by you and authorized staff of Dignity Health facilities. If you would like someone to have access to your records to help manage your care, you can sign a release to authorize a trusted relative to be set up as a proxy. This can also be terminated at any time by calling the technical support center. In the future we hope to add additional functionality that allows you to add additional information related to your health to your portal. If you do so, you'll have your information in a secure place that can be accessed anywhere you have Internet access.
The Portal serves as a central location for submitting submissions, viewing submission statuses, and reviewing communications from the FDA.
Portal will attempt to import your organization’s DUNS number automatically by performing a search in the Organization Information section. In some cases, DUNS number will have to be entered manually. So, please have this information available at the time of registration.
CDER NexGen Portal allows you to register Alternate Contacts and Partner Organizations (i.e. consortium, workgroups, etc.) to your DDT program. Alternate Contacts are individuals within your organization that will receive email notifications regarding updates regarding the DDT submission status.
When you use the Patient Portal Web Shortcut, you can communicate with your physician to stay on track with your care plans, monitor and share your health information, and request refills from within the app.
As soon as your record is created, you’ll be able to see your health data in MHS GENESIS.
