Online Medical Services Online Medical Services UCI Student Health Center is committed to exploring and implementing more effective and innovative ways to enhance the delivery of health care services to our students. SHC is please to offer a variety of convenient online services that are designed to increase and improve access to care for students.
Logon to the secure Student Health Patient Portal and upload the completed and signed TB Health Assessment Form for submission to SHC. Students who do not show risk factors for TB as indicated by their responses on the High Risk TB Screening Questionnaire are not required to complete/submit a TB Health Assessment Form to the Student Health Center.
This form is used by students who are covered by UC SHIP to request a referral to a community mental health provider. The form is submitted to SHC’s Insurance Services department who, in turn, will fax the referral authorization form to the designated provider’s office
This form is used when charges for services received at Student Health Center by a student, student-athlete, faculty or staff will be authorized by, paid for and recharged by SHC to another UCI department. The services and corresponding recharge to SHC must be authorized by a department official prior to the appointment. Refer to the instructions at the bottom of the form.
By having authorization to treat a student on file, students who are still 17 years of age or younger may seek treatment at Student Health Center without significant delays. This form is used to authorize the release of medical information from Student Health Center to a third party - not to the patient.

If you are a student or dependent enrolled in UC SHIP, you can request paperless delivery of documents (e.g., Explanation of Benefits) to your email directly rather than mailed to your permanent home address on file with the Registrar. Visit https://www.anthem.com/ca/ to request paperless delivery by email. For questions, please contact Anthem Blue Cross at (866) 940-8306 or to request a CCR form. If you are a non-student, previous student, outside medical facility, another healthcare provider, attorney, insurance company, or any other party, please download, print, complete and sign an Authorization for Release of Health Information.
To request medical records, you must submit an Authorization for Release of Health Information online via the SHC patient portal; in person; by fax; e-mail; or by mail. A reasonable, cost-based fee may apply. Applicable fees must be paid in advance before records are released.
Methods of Payment: Payment in the form of cash, check or credit card is accepted and can be made at the Cashier's Window in the lobby of SHC. If mailing a check, please include your name, student ID# and indicate that payment is for a medical records request. Please do not send cash. If paying by telephone using a credit card, please call the SHC Cashier at 949-824-7084. Copies of records will not be released nor will records from off-site storage be requested until payment has been received and confirmed.
All patient information is confidential and protected by privacy laws. Information from your records will not be released to anyone without your written authorization, except as required by law. Our Notice of Privacy Practices describes how medical information about you may be used and your rights as a patient.
Within the portal , students may add, update, edit and correct their health history. In addition, students may also enter allergies and medication information. Information entered by a student will appear in text in bold red font. A medical provider can resolve the student’s entry by reviewing and verifying the information with the student either through secure messaging or during a subsequent encounter/visit. The provider can then make the necessary edit to accept the entry.
Students who have terminated enrollment or graduated from UCI may continue to receive services at SHC for the same diagnosis for which they have started evaluation or treatment, on a fee-for-service basis, for up to one academic quarter after their last quarter of enrollment. Such students may continue to be seen at SHC in order to close the case or until outside referrals have been made, but for no longer than one academic quarter.
Non-registered students (e.g., extension students) are eligible for all services offered at SHC on a fee-for-service basis. Faculty and staff are eligible for urgent medical services and for pre-arranged occupational services with pre-authorized campus recharge. Faculty and staff may also utilize SHC to obtain immunizations and Pharmacy prescriptions. Dependents of registered students are not eligible for services at SHC.
Your student account will be billed. There is no processing fee for charges that are transferred to your campus student billing account. Visit Zot Account Online.
The Student Health Centers (SHC) at each University of California campus gladly offers reciprocal access to SHC services to currently registered students from all other UC campuses under the same conditions that apply to its own students who are not enrolled in its campus Student Health Insurance Plan (SHIP). Students enrolled in their campus SHIP may receive special discounts, benefits or services at their home campus Student Health Center. These special, campus-funded arrangements will not apply at another UC campus. (Refer to UC Systemwide Reciprocity Guidelines)
