What is the Northwest Health Patient Portal?

Northwest Health provides a Patient Portal help you be informed about your care and what you need to do to stay healthy. The Patient Portal is a secure, online home for your inpatient hospital health information.

How do I access my Northwestern Medicine patient account with mynm?

You can now access your Northwestern Medicine patient account multiple ways with MyNM,* powered by MyChart. If you are a patient of a Palos Health location, use the Palos Health MyChart to view your patient record.

What is the patient portal and how does it work?

The Patient Portal is a secure, online home for your inpatient hospital health information. In the Patient Portal, you can: View and share your personal health information related to your hospital stays.

How do I access information from the previous patient portal?

Previous Portal Users will need to create an account with MyHealthHome in order to continue accessing their personal health information. Information contained in the previous patient portal can still be accessed by contacting your local hospital. Click here for information on how to do that.

Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – Northwest Health will send an email to the email address you provided upon registration to the hospital. Follow the instructions in that message to create your account.

What is Northwestern Medicine?

Northwestern Medicine Regional Medical Group recognizes the need for patients to have complete access to care 24 hours a day, 7 days a week. We have nurses and physicians available to speak with you regarding your health concerns at all hours of the day. We strongly encourage you to use the MyNM Patient Portal to communicate with your provider care team for non-urgent matters. This method of communication allows you to store and retain your messages for later reference. It is important to stress that if you are experiencing chest pain, shortness of breath or another medical emergency, please call 911.

Where is the medical records department located?

Location: The Medical Records Department is located on the lower level of Northwestern Medicine Central DuPage Hospital. Use the Women and Children's entrance and stop at the Guest Services desk for directions. Hours: Monday through Friday, 8:00 am - 4:30 pm. Phone: 630.933.6361.
