To request the addition of a dependent (child) to your patient portal account

You must be the parent or legal guardian of the child/children listed below. Once a child turns 12, access will expire. Partial access can be granted for children ages 12-17. Once a child turns 18 all access will expire.

How to Request Your Personal Health Record (PHR)

A new Personal Health Record (PHR) must be requested every time you want to see any available updates to your health record. Please make your request at least 5 days after any test (s) were performed and only refer to the PHR sent to your Inbox for the results.
