Patients can create their own Nemours app account starting at age 12. Parents, legal guardians and patients can grant access to the Nemours app for other individuals by filling out the "Authorization to Disclose PHI to a Name Individual via the Nemours app." This form is available at Nemours and must be filled out in person.

You can use the Forgot? link next to the username field on the Nemours app login page to retrieve your username. You will be required to enter some identifying information, which must match your Nemours app account information. Once entered, the system will send an email with your current Nemours app user name.
You can ask your child’s provider to explain the test results to you by sending a message to the provider through the Nemours app or by calling the provider’s office.
1. Go to the App Store or Google Play . 2. Search for Nemours and download the app. 3. If you previously used MyNemours, just log in with your MyNemours username and password. 4 If you do not already have a MyNemours account, click "Create an Account" and follow the steps to create your account.
If you have forgotten your password, use the Forgot? link next to the Password field on the Nemours app login page to reset the password. You will be required to enter some identifying information, which must match your Nemours app account information. Once entered, the system will send an email with a link to reset your current password.
You can expect a reply to Nemours app messages within 3 business days. The Nemours app should not be used for urgent requests. Please call your child's provider for urgent matters, or call 911 in the event of a medical emergency.

To start utilizing NemoursLink submit the NemoursLink Practice Agreement for your practice or school district. Each individual user will also fill out the NemoursLink User Agreement. You can also call our Service Desk at (866) 736-2845 and request more information regarding NemoursLink.
This application allows specified non-Nemours health care providers, groups of providers, and/or authorized staff members of non-Nemours health care providers to view the information stored in these medical records. The clinician using this system will view this information using a connection from the NemoursLink Partner’s computer to Nemours computer network over a telephone line or the Internet.
The method of access uses a World Wide Web browser, such as Netscape or Internet Explorer, to display the information. NemoursLink is a secure, confidential and easy-to-use Internet based tool.
Delaware’s public school nurses see current medical information for students who come to them for care.
There is no direct charge by Nemours to access NemoursLink. The only costs would be those to update your computers, browsers or access lines.
Toll-free telephone support is available during standard business hours, Monday through Friday. This type of support offers unlimited access to support analysts that understand your issues and dependencies.
2. Each practice or school district must have a legal authorized representative sign the Practice Agreement form with Nemours to allow members of their organization to access NemoursLink. Individual users may not access NemoursLink without this document.
If you would like a physician or other individual or entity to have access to your child’s medical record, you must fill out, sign, and send (via mail, email or fax) an Authorization for Release Form (PDF). Identify who you’re authorizing under the “Disclose Medical Record To” section of the form.

If you think there is an error on your medical record, fill out, sign, and send (via mail, email or fax) a Request for Amendment Form (PDF) to the location you received care. Once your provider reviews the information, we’ll contact you with the outcome of your request.
You can revoke, or cancel, a prior authorization to access your child’s medical records by submitting your request in writing. Be sure to include the date of the release you want revoked, sign the letter, and send it (via mail, email or fax) to the location you received care.
