Signing Up for MyChart

All access to records through MyChart requires a MyChart account. Parents and guardians can be granted “proxy access” to the accounts for their children. Please select the category below that applies to you for instructions on how to sign up for MyChart:

I am older than 13 and would like my own account

Reach out to your provider's office and they can help you activate your MyChart account.

I am a parent of a child under 13 years of age

Reach out to your child’s provider and they can send you an enrollment link for a MyChart account with proxy access to your child’s records.

I am a parent of a child aged 13-17 years old

Reach out to your child’s provider and they can send you an enrollment link for a MyChart account with proxy access that will give you access to message your child’s care team and view or pay bills.

I am a parent of a patient who is 18 or older

In order to grant access to their account, a patient 18 years of age or older must create their own MyChart account by visiting MyChart.HopkinsMedicine.org and select “Sign Up Now” or by calling their practitioner.

I am a parent of a patient who is incapacitated

If you are responsible for the care of a person who is cognitively limited or otherwise incapacitated, please visit the MyChart Proxy Access page for details on how to gain access to MyChart for this patient.
