The Portal provides users with access to information and resources from MSK and allows for communications with healthcare providers and other MSK staff. Your use of the Portal does not create a patient relationship between you and MSK. The Portal is not a substitute for a medical consultation or medical advice, nor is the Portal intended to provide medical care or act as a replacement for visits with a healthcare provider. What is user information?

Patients may request a complete copy of their medical record by contacting MSK's Health Information Management department at 646-227-2089 or by going to the "Medical Info" section of the Portal, selecting "Ask for Your Medical Records," to complete a request form.
