Your use of the Portal does not create a patient relationship between you and MSK. What is user information?

The right you grant to MSK includes an unrestricted, worldwide, irrevocable, non-exclusive, perpetual, fully-paid, royalty-free, transferrable license. How to get a copy of my medical records?

Patients may request a complete copy of their medical record by contacting MSK's Health Information Management department at 646-227-2089 or by going to the "Medical Info" section of the Portal, selecting "Ask for Your Medical Records," to complete a request form.
