What to Bring to Your Appointment

Please bring current insurance information including referrals and insurance cards as well as a valid driver’s license or an alternate picture ID. Also bring any relevant medical records, x-rays or films. In addition, you may download and print the two forms below and bring them to your first appointment.

Patient Portal System

Our new patient portal system makes it easy to fill out and update your personal health information. Once you receive an email invitation from our practice, you’ll be prompted to create a user account for the portal. Updating your user account with new information instantly and seamlessly updates your electronic health record (EHR).

Billing and Insurance Information

We request payment at the time of service if you do not have office visit coverage, or if we do not participate with your insurance plan. Patients are required to pay deductibles, co-insurance, co-pays and non-covered services on the day of treatment. For our patients’ convenience, we accept Traveler’s Checks, Visa, MasterCard, and Discover Card.

After Hours Prescription Policy

It is our office policy not to call in prescriptions after hours or on weekends. Prescriptions and refills are handled during regular office hours.
