For medical records requests, please contact our Medical Records Department at Memorial Hospital – our team can be reached at (937) 578-2365 from 8:00am to 4:30pm, Monday through Friday. Alternatively, medical records requests can be submitted through MyChart.
Health Record Home patient portal is a free and secure online home for your inpatient hospital health information. The portal allows you to view your medical records on any internet-enabled device and become a more active partner in your healthcare. When you sign-up for Health Record Home, you will be able to:

Memorial is dedicated to helping improve your overall health care experience. We now offer the ability for you to securely connect some of the health management apps you may use (i.e. fitness trackers, dietary trackers, etc.) to your electronic health record.

Mail, flowers, and gifts are delivered daily to patient rooms by our staff of volunteers. Regulations prohibit live flowers in any Critical Care room. Mail received at the hospital after your discharge will be forwarded to the address provided on your admission form.
Providing superior care and service to each one of our patients and their guests is important to us at Memorial Hospital. At any point during your stay if you feel we are not living up to this expectation, the Patient Experience Department would welcome that feedback so we can make the necessary improvements.
Complete meals or snacks are available for your family and visitors in the Bistro ‘52 from 6:30 am to 6:30 pm, seven days a week. Vending machines throughout the hospital offer sandwiches, snack foods, candy, and beverages 24-hours a day.
Please send money, jewelry, and other valuables home with your family. If this is not possible, your nurse will arrange to have valuables stored in the hospital safe. The hospital is not responsible for money, jewelry, clothing, dentures, glasses, or other valuables kept in your room.
Memorial Hospital inpatients will each receive a copy of the local newspaper with their evening meal tray on Monday through Saturday. Additional copies are distributed in many of the department waiting areas.
Typically, this record contains your health history, current symptoms, examination and test results, diagnoses, treatment and a plan for future care or treatment. This information, often referred to as your medical record, serves as a:
You have the right to reasonable responses to any reasonable request s made for service. You have the right to know which hospital rules and policies apply to your conduct while a patient . You have the right to examine and receive an explanation of the hospital’s bill regardless of the source of payment.
At a minimum, the hospital shall include any persons living in your household and any support person pursuant to federal law.
