What is a care coordinator?

That is why we offer on-site Care Coordinators to help you navigate your healthcare needs. After an assessment by your physician, or a hospitalization, a Care Coordinator can be assigned to create a treatment plan with your physician to monitor any health needs. These Care Coordinators follow our patients along the continuum of an acute or chronic health problem, even assisting in scheduling follow-ups from hospitalizations or Emergency Room visits. What is chronic illness?

Chronic illness includes conditions such as hypertension , hyperlipidemia, diabetes, arthritis, and allergies.

Can a pharmacist monitor a medication?

Whether you are recently starting a new medication or have had a change in your medication regime, our pharmacist can support your medication therapy. Our pharmacist can also monitor complex medications such as warfarin, diabetes medications, and respiratory inhalers.

Can asthma be diagnosed with a physician?

Patients suffering from asthma are able to work with our physicians to find a treatment plan that works for them. Our physicians are able to diagnose the severity of asthma to provide long-term management for our patients.

FORMS FOR NEW PATIENTS

This form is for all new patients to fill out and bring to the clinic upon their first visit. Download and print out the blank form, fill out with a pen and bring with you to your first appointment.

FORMS FOR RETURNING PATIENTS

The Information Change form should be filled out when there is any type of change in the patient's information. From address changes to insurance policy number, etc. The entire form must be completed regardless of the information that has changed.
