A PIN is a unique number that we email to you to grant access to create your account. If you have not received a PIN by email please contact our office at (970) 879-6663. After receiving your PIN, visit Patient Portal to create your account by clicking the "Create Account With PIN" button. Once in the portal, click on the Let's Get Started ....
