Lexington Clinic now has a new patient portal with enhanced features! To access these features, patients, even those who were previously registered with Lexington Clinic’s patient portal, must register or re-register, using these easy steps.

In order to provide you with the best healthcare while protecting your privacy, we respectfully request that all communication with your Lexington Clinic providers be conducted via the Patient Portal or with a phone call to your provider’s office.

How old do you have to be to get a MyChart account?

Individuals must be age 16 or older to request a personal MyChart Account. Parents or guardians of minor patients, patient representatives, or other individuals may request access to a patient’s MyChart Account by following the terms outlined in the "Proxy Access" section of this Terms and Conditions statement. What to do if you need medical emergency?

If you are experiencing a medical emergency or need urgent medical care, immediately call 911, go to your local emergency room, and/or call your physician. MyChart is not your complete medical record. MyChart provides you with access to some (but not all) of the information contained in your electronic medical record.
