Laureate Medical Group is committed to providing an excellent experience to all patients. You will be receiving a survey via email after upcoming visits, and we would appreciate a few moments of your time to gather feedback. We value your answers and will keep them confidential.

Check in before you visit

Ask during check-in how you can streamline your arrival with our new pre-visit registration.

Phone Calls

Due to the high volume of calls, often you will reach our voice mail system. We assure you that all voice mailboxes are checked regularly and calls are returned promptly and appropriately during business hours.

Your Medical Records

Secure and reliable access to our patients' medical records is available through our on-line electronic medical records system. The relationship between the doctor and patient is private and any information exchanged will be held in the strictest of confidence.

Appointment Cancellations

In order to provide all our patients with the best care possible, we ask that you make every effort to keep your scheduled appointments and arrive at least 15 minutes early for your appointment. If you need to reschedule or cancel an appointment, we require a minimum cancellation notice of 24 hours.

Failure to Pay at Time of Service

All fees that are your responsibility are due at the time of your appointment. These include any co-payments, co-insurance, deductibles, or any other charges not covered by your insurance provider.

Patient Forms

We have invested in technology to better serve you. In lieu of registration forms, we ask that you complete registration via our mobile check-in option prior to your visit. This secure process allows us to maintain accurate, up-to-date information and results in faster check-ins.

The Northside/Midtown Medical Building offers parking rates for the general public and discounted rates for patients visiting the office

Please print your parking ticket and bring your parking ticket to the office suite front desk for your ticket to be validated or discounted.
