For new patients, please go to our Healow patient portal website (click the link) to fill out your new patient paperwork well in advance of your appointment. Please click the link in the bottom right to pre-register if you do not yet have an appointment and we will call you to set one up as you enter your demographics. Please also fill out the new patient intake questionnaire with your health information. This will help us to gather the necessary information and save you time at check-in as you arrive for your new patient appointment. Again, we are seeing new patients in the office with appropriate cleaning before and after each patient and social distancing. We ask that you arrive to the office 30 minutes prior to your scheduled new patient appointment to ensure we have adequate time for paperwork and computer input.
