You can use your MyChart account to request proxy access. For access to patients age 17 and younger, use your adult account to request access to a child's records. Adults (age 18 and older) can use their MyChart account to request another person to access their MyChart information to help with their health care.
In order to grant access to their account, a patient 18 years of age or older must create their own MyChart account by visiting MyChart.HopkinsMedicine.org and select “Sign Up Now” or by calling their practitioner.

When the patient is age 12 or younger: Parents/guardians can request a proxy account and will have full access to the child’s MyChart information. For patients ages 13 –17: Parents/guardians can continue to use MyChart to share messages with some of their providers and view or pay bills.
Parents or guardians accessing information about a patient ages 13-17 will only have limited access to MyChart, with the ability to message some of their providers and view/pay bills online. This limited access reflects privacy laws and encourages teens to feel comfortable sharing health concerns with their care team. Teens aged 13-17 are encouraged to establish their own MyChart account, where they can see their full information.
