Greenway Health Patient Portal

Before your initial appointment you will receive an email with instructions on how to access this secure web site. Completing your medical history survey on-line via the Patient Portal will allow us to provide you with more prompt and comprehensive care.

Greenway Patient Portal is a free, password-protected and web-based interactive service that allows you to securely access and track your health information. Using your personal computer or smart phone, you can sign in to Prime Patient Portal and see your test results, medication you are taking and a summary of your current health conditions

How do I sign up? During registration you will be asked for your personal email address. Within a few days, you will be sent an email with a link to the Greenway Patient Portal web page. Once you click on the link and follow the prompts to set-up your account, you will receive and email with your username.
