Students who would like to obtain a copy of their immunization history at Perimeter College at Georgia State University and entered Perimeter College at Georgia State University after spring 2015 can obtain their immunization records through the Patient Portal. Office visits for Student Health are currently scheduled by appointment only. Patients arriving 10 minutes past their scheduled appointment time will be asked to reschedule.
Students should have a copy of his or her insurance card with them. An available copy of your insurance card can be attached to your medical history form for information only. The clinic does not provide billing for insurance. The clinic does not treat auto-accidents or workman's compensation due to insurance requirements.
In the event you suffer an illness or injury that requires you to miss class, student health will provide you with discharge paperwork that provides specific discharge instructions regarding absence and return to class. We do not fill out disability paperwork or paperwork for employment.
Emory University Student Health Services (EUSHS) provides outpatient care for enrolled Emory students with a valid Emory ID card. International student's spouses, Domestic Partners and unmarried children over 18 years of age are also eligible for primary medical care if they are currently enrolled in the Emory/Aetna Student Health Insurance Plan.
