New Prescriptions

Most new prescriptions are submitted electronically to your preferred pharmacy by the end of the day of your appointment. You can confirm if/when your prescription has been submitted and verify which pharmacy we have on file by checking the patient portal.

Refills

If you have a prescription that needs to be refilled prior to your next office visit please submit a request through the patient portal. Using the patient portal for refill requests is the most efficient way to ensure the accuracy of the prescription you need refilled.

Schedule II Controlled substances

Some Schedule II substances such as Adderall, Vyvance and Vicodin can be useful for controlling symptoms of ADHD and pain but have a high potential for misuse and are, therefore, closely controlled by local, state, and federal governments.

Appointments

In wake of the COVID-19 Pandemic, we continue offer Telehealth video appointments for quick conversations, such as follow-up appointments, reviewing medications or test results, etc. Our schedule has been changing almost weekly as we respond to circumstances, so please use the booking buttons below to check availability.

Office Visits

There are some types of appointments for which Telehealth is not practical. For patients that do need be seen in-office, we are taking the following steps to reduce exposure to other patients:

communication

During this pandemic we are committed to doing everything possible to ensure our patients receive the care, medication, and information they need. In order to help us accomplish this, we ask that you help minimize the influx of phone calls by using the patient portal whenever possible.

Billing Questions

While we use a third party billing company, some basic billing questions can be answered by our front desk at 773-832-8700.

Clinical Inquires

Instead of calling or emailing the front desk to inquire about clinical concerns such as refill requests, or the status of test results, we strongly encourage patients to message the clinical team directly from the patient portal. In addition, please see the links below that provide answers to common questions:

That the results warrant a relatively straightforward and simple action (i.e sending a short term prescription or recommending an over-the-counter medication) the provide…

and If You Still Have Questions?

  • Start by sending your provider a message via the patient portal. Please note however, that while we strive to provide the most convenient methods for patients to communicate with their clinical provider, efforts to respond to even “quick questions” via the portal can at times prove to be cumbersome and inadequate for both patients and our staff. Fortunately, most insurance plans r…
