Proxy Authorization Forms

A Proxy Authorization Form is required to access the health information of anyone over age 12 by someone other than themselves. Please note that all signatures must be witnessed by an FHN staff member.

What is the Northern Illinois Food Pantry?

FHN and the Northern Illinois Food Pantry are helping get a healthy “dose” of good food to those who need it. This free program will distribute healthy produce, milk, eggs, and frozen and canned meats to qualified participants on a regular basis.
