To create an account in the Patient Portal, call us at 912-354-4800 or visit us and our expert staff can help you. We will create a NextMD account for you and give you a personal enrollment token (an 8-digit number used to sign in). Once we create your account, you will receive an email with instructions on how to enroll in the Patient Portal.

How to access the Patient Portal?

The Patient Portal is a website through which you may communicate with our office. Registered patients would have secured and confidential access to the website and may: 1 Ask a question 2 Receive a clinical summary of your office visit 3 Receive results, education material and information online 4 Request an appointment 5 Request a medication refill

Patients can now pay with ACH (must include routing number and account number or send check to address on statement).

Colorado Eye Institute welcomes you to our practice! To decrease the amount of time prior to your appointment, we recommend that you complete the new patient exam forms from this page and bring them with you to your exam.

Medical Records Release Forms

In order to release a patient’s medical records we need to have the following form filled out. The form must be signed by either the patient or their legal representative, electronic signatures are not accepted. The form can be submitted in officer or via fax.
