A physician is on call 24 hours a day. In an emergency after regular office hours, call the main number and the answering service will contact the physician on call to help you. If it’s a life-threatening emergency, call 911.

What to bring to a med appointment?

All patients, new and established, should always bring the following to your appointments: 1 Current health insurance card and photo ID. 2 Preferred form of payment. 3 Information about any changes in the medications you’re taking, or stopped taking, including over-the-counter medications, vitamins, and herbal supplements. 4 Information about other changes such as new phone numbers and medical history updates.

How early should I arrive for my first visit?

For your first visit, please arrive about 15 to 20 minutes early to fill out patient information forms and register as a new patient. If you’d like, you can download the forms and complete them before you come in. It’s essential that we have enough time to care for you during your visit.
