How do I get patient portal access for my adolescent patient?

Adolescent patients are NOT provided patient portal access. Their parent/guardian must call 315-463-1600 and speak to CNY Family Care’s office staff. The staff will request some personal information (both the adolescent patient and the parent/guardian’s names, address, phone number and insurance information).

Who is the owner of the patient portal?

Our Patient Portal is owned and operated by Medent/CCS and is fully compliant with federal and state laws concerning privacy for each patient. A patient’s name and e-mail address will be treated with the same care and privacy given to your health records and will never be sold or leased by CNY Family Care or Medent/CCS.

What is the patient portal in medent?

The Patient Portal in Medent is a personalized, secure on-line way for patients to gain access to their individual medical record. It is a FREE service that we offer to our patients which enables each patient to securely use their Internet connection to help manage and receive information about his/her personal health.

What can I talk about in the patient portal?

The patient portal is meant to talk about your medical information with your medical team only. If you need to discuss another patient, please have that patient use their patient portal account or contact our office. Per federal CDC guidelines, WE ARE STILL REQUIRING ANYONE ENTERING THE OFFICE TO WEAR A MASK.

How to access patient portal?

To access the PATIENT PORTAL, select the link at the top right of the page. The PATIENT PORTAL TAB in the blue ribbon at the top of the page is for assistance with getting onto our Patient Portal.

Do you have to wear a mask at CNY?

CNY Family Care requires all patients entering our practice to wear a face mask or cloth face covering . Please be sure to sanitize hands upon entering and leaving the Practice and follow social distancing of at least 6 feet apart from others when others are in the waiting area.

Is it important to have access to a primary care physician?

We feel that access to your primary care physician is one of the most important aspects of good healthcare. In an effort to accommodate as many of our patients as possible, we offer extended office hours during the work week and also on the weekends.

Does CNY Family Care have Pfizer?

We do have the Pfizer booster at CNY Family Care. If you are choosing to receive your vaccine booster through CNY Family Care, and not through your local pharmacy or health department, you must call the office for an appointment in our vaccine clinic which is held every Saturday and Sunday. Please be aware, at this time we are scheduling into November at our adult vaccine clinic for both a flu shot, a Pfizer Covid-19 or a Moderna booster.
