How can I keep messages sent through the patient portal secure?

New Patient Forms. If you are able to download and complete the new patient packet prior to your appointment, you are able to expedite your check-in process on the day of your appointment. If you cannot fill the forms out before your arrival, please allow 30 minutes to complete the necessary paperwork.
