How do I change my personal information?

If you notice an error in your personal information, such as address and phone number, please send us a change request through the portal. The change request can be found by clicking Profile, then Update Profile.

What if I need help accessing my electronic health records?

What if I don't have an email address?

There are many free email services available. Any email service is compatible with the Phelps Memorial Patient Portal. A parent or legal guardian must sign the release for a child or mentally incompetent patient, with required documentation. We require at least 24 hours, and not to exceed 30 days, to release medical information, unless it is a medical emergency.

We very seldom have blood types available, as blood types are not routinely done at PMHC unless you have had surgery where a significant blood loss is possible . For babies' blood types, if you gave birth at PMHC, you would have been informed by the nurse or received a card noting the blood type for you to keep with other birth information. If not, you must follow the same requirements for Release of Information, as stated above.
