Results: Patient portal users tended to be older (49.45 vs. 46.22 years in the entire sample, p = 0.008 in the random sample) and more likely female (62.59 vs. 54.91% in the entire sample, p = 0.035 in the random sample). Nonusers had more monthly emergency room (ER) visits on average (0.047 vs. 0.014, p < 0.001).
Methods: A qualitative comparative case study was conducted to determine the feasibility, acceptability, and initial perceptions of users and to identify ways to increase uptake. Semistructured interviews were conducted with 56 participants (27 patients, 21 providers, 4 nonmedical staff, and 4 clinic managers) from 5 clinics.

BVCHC plans to evaluate the patient portal implementation through patient satisfaction surveys and by comparing responses between groups of registered portal users and patients who are not using this resource. As portal enrollment and use increase, BVCHC is also planning to examine whether use of the portal results in a reduced burden on the BVCHC call center.
Medical assistants promoted the patient portal as a way for patients to establish a direct line of communication with their provider. Patients registered with the portal can avoid using the BVCHC call center, which receives a large volume of calls.
Limitations of the selected EHR and patient portal, including secured messaging communications with patients in languages other than English, with the exception of patient education materials.
Facilitating the subsequent implementation of the NextGen Health Information Exchange (HIE) among other area practices also under the NextGen Umbrella to enhance continuity of care for patients and provide a common engine and system interfaces for use by participating practices, local laboratories and imaging facilities, hospitals, and consulting providers within the NextGen HIE network.
A message is instantly delivered to the patient’s e-mail address encouraging him or her to complete the second step of the patient portal enrollment process. Processes are also being put in place to allow patients to complete enrollment while on-site during a visit.
BVCHC found it helpful to designate point persons for portal enrollment, such as medical assistants, to engage patients one on one. Consequently, providers are not burdened with the enrollment process, but they can encourage patients to use the portal. Moreover, assigning a dedicated triage nurse to serve as the gatekeeper for messages coming through the portal has eased provider concerns about email volume and time required for patient communication.
Patient-Specific Education Resources.#N#EHR has an integrated patient education tool that allows clinical staff to search and select from more than 600 summaries on diagnoses and symptoms and more than 1,000 medications. Materials, which are available and English and Spanish, can be printed out and reviewed with patients at the time of the visit.
The clinic has an online platform, MyWellness patient portal, that allows patients to play an active role in their own health care. On the portal, patients can communicate with their doctors, make appointments, check lab results, order medication, and access their health information even when the clinic is closed. The online portal allows even those patients without cell phones or computers to communicate with their health team from a public library. The problem? Few of the clinic’s patients were using the service: as of January, 2018 only 4 percent were enrolled in and active on MyWellness patient portal.
At first trainings stressed the benefits of increased portal use: 1 Enhanced patient/medical team communication 2 Improved patient awareness and management of health conditions 3 Fewer patient phone calls and walk-ins

of the portosystemic stent. While portal venography is excellent at demonstrating stent patency, Doppler ultrasound allows visualization of blood flow within the stent, portal system and hepatic venous system without the need for radiation and contrast injections. In December 2018, Dr. Abhat presented the outcomes to the DHS Executive Clinical Operations Committee and the California OpenNotes Consortium.
It was lunch time, which meant the Medical Records staff had closed the window and the weary people in line could expect an extra hour’s wait. Some were on crutches; others had babies and children in tow. There was nowhere to sit down, and patients were frustrated, Abhat recalls. “As usual, everyone in the line was angry and annoyed,” she said. “It definitely gave me pause.” And that’s how the idea of a new way for patients to get their medical records was born.
First, they spent time explaining what OpenNotes was. It may sound like an app or software “or someplace in the sky where medical records are stored ,” as one staff member said, but it’s not: It’s a movement to make healthcare more transparent “by urging doctors, nurses, therapists and others to invite patients to read the notes they write to describe a visit.” In other words, Open Notes. As one radiology journal recently pointed out, studies have linked this practice to “improvements in patients’ understanding of their health, promotion of shared decision making and patient-provider communication, and, ultimately, improvements in patient outcomes.”
The nation’s second largest municipal health system, the LA County DHS cares for more than 600,000 patients annually, 80 percent of whom receive public insurance through Medi-Cal. The organization is a leader in trauma and burn care as well as chronic disease management. No one is turned away: Its doctors, nurses and other employees work around the clock to take care of patients who are low-income, uninsured, homeless, speak little or no English, and/or have disabilities or substance abuse problems.
In the original evaluation and demonstration study on OpenNotes, conducted at health systems in Washington, Pennsylvania and Massachusetts in 2010, researchers found physicians feared that turning over their medical notes to patients would result in an additional burden on providers.
Dr. Abhat was one of the very first doctors to use OpenNotes because she was part of the original pilot study on it when she was a provider at Harborview Medical Center at the University of Washington in Seattle.
There were still lines for Medical Records, but they were smaller. Meanwhile, physicians reporting hearing moving testimonies from their clientele about the new technology.
