Please be aware that the Patient Portal may not be monitored by your healthcare provider outside of normal business hours.
Visit the My Health Profile patient portal homepage . Select Enroll Now. Enter your information and click Submit . Once submitted, you will receive a notification code to the cell number provided. Enter code and click Submit. Enter your email address and create your password. Agree to consent forms and click Submit. You are now enrolled!
