How do I request medical records from Avance care?

Fill out a written request at your closest Avance Care location Mail request to: Avance Primary Care, P.O. Box 90759, Raleigh, NC 27675 Please note that signed consent is required for all medical records requests. Please make sure to send a release with all medical records requests. Please allow 5 – 7 business days for processing.

When is the medical records department open?

If you are 18 or older, you can complete an authorization form in writing or online. The medical records department is open 7:00 AM – 4:00 PM, Monday – Friday.

Can you fax a behavioral wellness record?

Please note that these records can only be faxed or mailed and cannot be picked up from the office.
