At Meridian Nursing and Rehab, we understand that no one looks forward to entering a skilled nursing facility, even if it is only for a short term stay. The role of our Admissions Department is to assist families who need to find immediate placement for a loved one. Often, a Discharge Planner or Social Worker at a hospital informs a patient’s primary contact of the need for short term rehabilitation or long term care placement. Once this decision has been determined, families must quickly choose a facility.
