At Meridian Nursing and Rehab, we understand that no one looks forward to entering a skilled nursing facility, even if it is only for a short term stay. The role of our Admissions Department is to assist families who need to find immediate placement for a loved one. Often, a Discharge Planner or Social Worker at a hospital informs a patient’s primary contact of the need for short term rehabilitation or long term care placement. Once this decision has been determined, families must quickly choose a facility.
The agreement between Meridian Nursing & Rehabilitation Center and the Secretary of the Department of Health and Human Services will be terminated on August 14, 2021 in accordance with the provisions of the Social Security Act.
Original Medicare will generally pay for short term rehabilitation up to 100 days (lifetime) after a hospital stay of at least 3 inpatient days or certain other circumstances after you meet your deductible. Long-term nursing home care may be paid via Medicaid after long-term care insurance and personal savings are used.

The family is educated on the patient's limitations and strengths and how to help the patient continue functional improvements at home.
