Two Ways to Create Your MyHealthHome Account

Enrollment Invitation – AllianceHealth Durant and Madill will send a text and / or email to the email address and/or cell phone number you provided upon registration to the hospital. Follow the instructions in that message to create your account.

A COVID Vaccine pop-up event is being held Saturday February 5. We will be administering the Pfizer COVID vaccine for ages 5yrs and older.

Covid Vaccination Update

Alliance’s community-wide Covid-19 vaccination program comes to an end on 1/31/22. The last first dose for non-patients will be on 1/10/22. We will continue to provide 2nd and booster doses through 1/31/22. Use the scheduler here or call us at 1-707-433-5494 to schedule.
